2025 CHADD Volunteer Leader of the Year Award Nomination Form |
Welcome to the nomination process for the CHADD Volunteer Leader of the Year Award!
Volunteers are the heart of our community. They drive change and make a difference through their selfless dedication. This award recognizes an outstanding local volunteer who has made exceptional contributions to the CHADD community, and in their own local community, during their involvement with CHADD.
When submitting a Volunteer Leader of the Year nomination, you may consider chapter board members, chapter coordinators, or chapter volunteers – any of whom you feel made exceptional contributions to the CHADD community in the past year. To be eligible, nominees should be members in good standing of CHADD and active participants of a local CHADD chapter. The activities of the volunteer should align with CHADD's mission of providing evidence-based ADHD education, peer support, and advocacy. Consider aspects such as leadership, innovation, character, and program outcomes.
Please note that CHADD National Board Members, Professional Advisory Board Members, current members of the Chapter Advisory Board, and paid CHADD staff are not eligible for nomination.
Please complete this form in its entirety for consideration.
Deadline to Submit: May 19, 2025.
